MOC’s Grassroots Gala Nomination Form
This is the second, of what will be an annual event to celebrate an individual or group who has made a significant commitment to providing support to our community in a manner that improves the ways and spaces where we live, work, learn and play.

We are looking for community members to highlight and recognize for the following awards:
Community Advocate Award
The advocacy recognition will honor an individual who has played a lead role in developing or growing the Gainesville (or any Alachua county) community by working to inform decision makers, the public and the media and/or affecting policy and legislation through effective advocacy at the local, state and/or national levels. This person must be an authentic role model within the community who exceeds normal expectations of regular advocacy practice and who is inclusive and respectful of the values and personal autonomy of those who are affected by it.
Community Educator Award
This recognition honors a member of Alachua County who through their service has enhanced and advanced the education and lives of others in our community as evidenced by their work’s quality, quantity, and creativity. The individual recognized is someone who continuously encourages, innovates, challenges, and empowers others, in formal or non-traditional learning environments, to become positive community members. They impart not just knowledge but the continuing love of learning and discovery onto individuals around their community.
Community Care Award
The social support recognition will honor an individual who demonstrates a passion for diversity and civic engagement and shows initiative in responding to social needs. This person enhances the well-being of people within the community by helping them meet their basic needs, with particular attention to the needs and empowerment of people who are vulnerable, oppressed, and/or underserved. This person also shows a drive for social change and promotes equity across race, ethnicity, creed, religion, gender, socioeconomic status, sexual orientation, disability, and physical and mental challenges.
Ginny Bruzzese Service Award
The Ginny Bruzzese Service Award will honor an individual who encompasses every aspect of the above award categories for at least two years. The recipient of this recognition demonstrates an unparalleled love and passion for serving the Gainesville and Alachua County communities and its members through advocacy, education and social support. This person is the pinnacle of community service and stewardship, living life with the goal of improving access and support for all in their communities.
