Our care is informed by our assessment of the social determinants of health (SDOH) for each of our patients. After their initial appointment, patients are contacted by one of our care coordinators to assist them in creating a customized solution to overcome any barriers that were identified during their SDOH assessment.
- Use available data to identify neighborhoods where residents have a lack of access to healthcare.
- Work with and train medical and PA students on how to provide SDOH informed health assessments.
- Train UF Student care coordinators on how to identify existing community resources to address the needs of our patients.
- Provide disease prevention services with community partners.
The Mobile Outreach Clinic provides primary medical care services at no cost to the patient and will provide services to all individuals regardless of their ability to pay or because of their race, color, sex, national origin, disability, religion, or sexual orientation.
