The Mobile Outreach Clinic provides primary medical care services at no cost to the patient and will provide services to all individuals regardless of their ability to pay or because of their race, color, sex, national origin, disability, religion, or sexual orientation.
Services offered by MOC
The Mobile Outreach Clinic offers a variety of services for our patients. These services are listed below:
Primary care services typically include:
- Routine physicals and wellness check-ups
- Routine health screenings, including:
- Blood pressure/sugar reading and monitoring
- Confidential HIV counseling and testing
- FIT test (self-collected screening test for colon cancer)
- Reproductive health screenings
- Pap smears/pelvic exams
- Breast exams
- Pregnancy testing (hCG)
- Family Planning and contraceptive care education and support
- Administration and education of Long-Acting Reversible Contraceptions, including:
- IUD and Nexplanon (implant) insertions (for uninsured individuals)
- IUD and Nexplanon (implant) removal
- Check-ups for minor acute ailments and illnesses
- Care for chronic illness including:
- Hypertension (high blood pressure)
- Diabetes
- COPD (Chronic Obstructive Pulmonary Disease)
- Depression
- Back pain/chronic nerve pain
- Referral services
- Including but not limited to WE CARE Network and UF Health specialty clinics
- Administration of medications, over the counter (OTC) and prescription drugs (excluding narcotics and other controlled substances)
Specialty services:
- Pharmacy team consultation
- Diabetes consultation and education,
- Medication education/reconciliation
- Supportive chronic disease management (hypertension, diabetes, high cholesterol, etc.)
- Consultation and management of Patient Assistance Program applications for need-based free/reduced-cost medications
- Clinical psychology
- Neurology
- Ultrasound
Other services available through the Mobile Outreach Clinic includes:
- Care Coordination
- Regular follow up by a trained patient advocate to help support patients’ health and medical goals by offering social services and connection to various community resources
- Facilitation of chronic disease self-management courses
- Community health work education
